Enrolling in Care N’ Care is easy. Just follow these steps:
1. Meet eligibility requirements
To be eligible for Care N’ Care, you must be entitled to Medicare Part A and enrolled in Medicare Part B AND live in Tarrant County.
Some enrollment restrictions apply.
2. Submit an enrollment request
By phone
You can enroll by contacting Care N’ Care at 817-810-5213, TTY users should call 817-529-5268 from 8:00a.m. to 8:00p.m. seven days a week.
By mail
Print a 2009 Care N’ Care Enrollment Form, fill it out and return it to:

In-home appointment
You can contact a Care N’ Care Benefits consultant directly at 817-810-5213 and make an appointment, TTY users should call 817-529-5268 from 8:00 a.m. to 8:00 p.m. seven days a week. A licensed Benefits consultant will visit you and provide all the information that you need to make the best decision.
Medicare beneficiaries may also enroll in Care N’ Care through the Centers for Medicare and Medicaid Services Online Enrollment Center located at www.medicare.gov/. You may be able to get extra help to pay for your prescription drug premiums and costs. To see if you qualify for getting extra help, call:

Enrolling outside of the Annual Election Period (AEP) or the Open Enrollment Period (OEP)
Have you recently become entitled to Medicare Part A and Part B for the first time?
If you have recently become entitled to Medicare Part A and Part B for the first time, you may enroll within the three months before, the three months after or the month of entitlement. This seven-month window is known as your Initial Coverage Election Period (ICEP). If you wish to join after the Initial Coverage Election Period, you will have to wait until the next Annual Election Period or Open Enrollment Period. You may be charged a Medicare-required late enrollment penalty on your future premiums if you did not previously have other creditable coverage. Avoid this late fee by enrolling as soon as you are first eligible.
Special Election Periods (SEP)
There are certain circumstances that would qualify you for a Special Election Period, that is, enrollment outside of the Annual Election Period or Open Enrollment Period. For instance, if you move into the service area or involuntarily lose your current creditable coverage, you would be eligible for a Special Enrollment Period. Care N’ Care is a Preferred Provider Organization (PPO) with a Medicare contract that is renewed annually, and the availability of coverage beyond the end of the current year is not guaranteed. You are eligible to enroll in Care N’ Care if you are entitled to Medicare Part A and enrolled in Medicare Part B and you live in Tarrant County, Texas. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. With some exceptions you can only enroll in Care N’ Care during certain times of the year, for more information on enrollment periods please contact a local sales representative. With the exception of emergency or urgent care, it may cost more to get care from non-plan providers.
