General Questions

Updated Medicare Advantage (MA) requirements, as well as increasing market pressures, resulted in an untenable environment for smaller MA plans like Care N’ Care. As a result, Care N’ Care (HMO/PPO) elected not to offer a MA product in 2025.

Medicare disenrolled the member to Original Medicare unless they chose to enroll in another Medicare Advantage plan first. The former member (now beneficiary of Original Medicare) has a special election period (SEP) through 2/28/25 to enroll with another Medicare Advantage Plan. They must contact that plan or a sales agent to request enrollment. Alternatively, they can go to Medicare’s Online Enrollment Center and submit an enrollment request.

Medical and Pharmacy benefits ended midnight 12/31/2024.

CNChealthplan.com will remain up through 12/31/25. The Customer Experience team will also be available through 12/31/25.

Payments / Claims

Members are responsible for their plan premiums for any month they were enrolled on the plan. This does not include benefit runout months in 2025.

There will be no change from existing processes. CNC will accept claim reimbursement requests up to one year after the date of service. Services paid by members on 12/31/2024 may be billed up to 12/31/2025.

This is currently anticipated to be 12/31/2025.

In 2024 members in the Southwestern Health Select HMO Plan were given $800 annually in Flex benefits to cover out-of-pocket costs for dental, hearing, and vision supplemental benefits. Any unspent Flex benefits expired at the end of 2024.

In 2024 members in the Southwestern Health Select HMO Plan could earn up to $750 annually in Rewards benefits for completing certain preventive health screenings and wellness activities. Rewards benefits were accrued as earned. As Care N’ Care will continue to process runout of claims in 2025 for services received in 2024, some members will earn rewards in 2025. Any unspent Rewards benefits will expire at the end of 2025.

CNC is responsible for the services the member receives until discharge.

Portals

We anticipate supporting the member portal throughout the runout period until 12/31/2025.
