Care N’ Care Medicare Advantage Health Plan closed effective December 31, 2024, and the one-year runout for filing a claim ended December 31, 2025.  This means that claims are no longer being accepted or processed.  In addition, the Care N’ Care Customer Service Department closed December 31, 2025, and is no longer taking calls.

If you are a former Care N’ Care member, member’s appointed representative, or a non-network provider who would like to file an appeal regarding a claim that was denied within the last 65 days, please email Yourteam@cnchealthplan.com and include the following information in your email: 

  • Member Name
  • Member ID Number
  • Member Date of Birth
  • Claim Number
  • Claim Date of Service
  • Provider Name
  • Provider National Provider Identifier (NPI)
  • Provider Tax ID Number (TIN)
  • Billed Charges
  • Claim Denial Reason
  • Your Reason for Appealing the Denial

Non-network providers must also submit a completed Waiver of Liability Statement for their appeal to be reviewed.

Timely filing rules will apply to all appeals.

If you are a former Care N’ Care member or member’s representative who would like to file a grievance related to something which occurred within the last 60 days, please email Yourteam@cnchealthplan.com and include the following information in your email:

  • Member Name
  • Member ID Number
  • Member Date of Birth
  • Your Email Address
  • Your Telephone Number
  • Date of Event Occurrence
  • Your Reason for Filing the Grievance
