Claims & Payments

CNC will accept and process claims through 12/31/2025. Timely filing deadlines still apply.

There will be no changes to existing processes. CNC will continue to accept claims throughout the runout period ending 12/31/2025. Our EDI payer ID will remain 66010. Paper claims should be sent to:

Yes, EFT will continue to be available the process will remain the same in 2025.

Call Provider Support at 844-806-8216 or log into your Availity provider Portal to view claims status details.

Authorizations

The last day to submit an authorization request was 12/31/24 with the exception of in-patient admissions.

Yes, Providers will have access to Acuity Connect until August of 2025.

General

After 12/31/24, we will not be adding any new providers.

If you need to update a provider’s demographic information, please go to our website and fill out a Provider Demographic Information Update form: https://cnchealthplan.com/providers

NOTE: Demographic updates are needed only if they will impact processing of claims from 2023 or 2024.

No recredentialing is required after 12/31/24. If you have any questions, please contact our credentialing vendor, Medallion, at support-swhr-care-n-care@medallion.co

Yes, the Care N’ Care website will continue to have a Provider section with the foundation information for providers.
